How did it happen? Perhaps we'll never know.

A prisoner has been released from Wandsworth prison "in error".

Ryan Byrne, 34, is wanted by police who were told about the blunder on Friday.

He is 5ft 11 ins tall, of medium build, with blue eyes and a goatee beard.

It is not clear how the error happened.

And it is not the first time Wandsworth has been embarrassed.

Neil Moore, 28, collected his belongings and walked out of Wandsworth Prison on March 10 last year, having set up a fake website and sent emails “from the courts service” to fool the prison into thinking he had been granted bail by a judge.

The prison is also facing a serious inquiry into the number of deaths inside - three times the number which could be expected, according to a coroner.

Anyone who has seen Byrne is asked to call police on 101.
