Passed away peacefully after a long illness on 7th January 2009 aged 39.
He will be greatly missed by his wife Sarah, daughter Ellie and his family and friends.
Funeral Service to take place at St. Dunstan's Church, Cheam on Monday 2nd February at 1.00pm followed by Committal at the North East Surrey Crematorium, Morden.
