A report into the death of a father and daughter in a speedboat crash has found the driver failed to attach a kill cord which could have saved them.
Mr Milligan, 51, from Routh Rd, Wandsworth was holidaying in Cornwall with his wife Victoria and four children on May 5 when the family was thrown from their speedboat Milly.
The BSky B executive was killed alongside eight-year-old Emily, while the other four all sustained injuries as the unmanned vessel span out of control performing a number of tight turns at high speed.
The Marine Accident Investigation Branch (MAIB) published a report this morning stating the boat was fitted with a kill cord, designed to immediately cut power to the engine, but this was not attached to the driver, and so was not pulled when the Milligans were thrown overboard.
A full report will be published by the MAIB following further investigation.
