Three patients looked after by the trust that runs Tolworth Hospital committed suicide in the last three months of 2012-13, a report has revealed.

South West London and St George’s Mental Health Trust (SWLSTG) recorded a total of 28 “serious incidents” in the final quarter of the last financial year.

The trust runs mental health and drug addiction services in the region, and admits about 3,500 inpatients a year over five main sites.

The incidents were revealed in a governance report presented to Kingston clinical commissioning group’s (CCG) governing board meeting on Tuesday.

Two outpatients also tried to take their own lives, the report said, while there were 11 unexpected deaths among community patients.

There were two leaks of confidential information, and two instances of “sub-optimal care of the deteriorating patient”, while there were eight further uncategorised incidents.

CCG governing board member Vanessa Lodge said at the meeting there were “concerns” over the incidents.

She said: “We will be asking south-west London and St George’s to present to us on a range of issues in terms of their own internal quality assurance. It needs to come to the attention of this board.”

Councillor Margaret Thompson, chairman of Kingston Council’s health overview and scrutiny panel, said: “If we have serious untoward incidents that chime with a systems problem then we would be looking at the fundamental cause that allowed that to happen.

“We can’t look at individual cases – we don’t tend to bring them into health overview because of confidentiality.”

According to the Care Quality Commission’s most recent checks Tolworth Hospital met standards in five key areas including management and protecting patients from harm.

In the same period Kingston Hospital recorded eight serious incidents including two cases of bedsores, two delayed diagnoses, and a patient suffering an accident at hospital.

A spokesman for SWLSTG said: “Whenever an incident is reported, SWLSTG always investigates fully to ensure we are providing the best possible care to our community.

“The trust has taken action to strengthen our systems and ensure the timely and detailed reporting of these incidents.”
